If you have a birthday party booked with us please complete and submit this form so that we can make sure to have everything prepared your party. To book a birthday party click here to view our bookings calendar.

We will email you 10 days prior to your event date to confirm all of the information on this form. At that time, we will ask for an updated headcount and you may also change your selections and/or quantities.

You are welcome to bring outside food and drinks (non-alcoholic). We also offer two food package options for your entire party where we order all the food and drinks for you! You may also select individual add-ons towards the bottom of this form.

1. Pizza Package ($225):

Three extra-large 18" cheese pizzas
Fresh fruit platter
Honest Kids juice boxes for all kids (up to 16)
La Croix sparkling water for all adults (up to 32)
All drinks come in iced drink buckets
2 dozen cupcakes -or- 4 dozen mini cupcakes
Birthday-themed paper goods for all your guests
Birthday-themed tablecloths (2)

2. Breakfast Package ($165):

Mini continental breakfast platter (26 piece)
Fresh fruit platter
Honest Kids juice boxes for all kids (up to 16)
Juice boxes come in iced drink buckets
2 dozen cupcakes -or- 4 dozen mini cupcakes
Unlimited coffee & hot tea bar
Birthday-themed paper goods for all your guests

Food packages include two birthday-themed tablecloths. Tablecloths are optional and not required in our party room. Please indicate in the next question if you would like us to set out tablecloths for your party. Click here to see the tablecloths.

Please note: there are no substitutions allowed for our food packages but you are welcome to include other add-ons or increase quantities for an additional cost.
